What to report
SARS-COV-2 infections/COVID-19 cases, hospitalizations, deaths, and test results must be reported to MDH.

How to report COVID-19/SARS-CoV-2 cases/infections
Case report forms for health care (e.g. health care providers, clinics):

The provider portal allows for submission of multiple case reports using spreadsheets or flat files. - Phone

Deaths from COVID-19 are reportable to MDH.

Report form for hospitals:
Hospitals are required to submit reports on hospitalizations for patients who had a positive viral test for SARS-CoV-2 in the 14 days prior to admission or during their hospital stay.
- COVID-19 Provider Portal
The provider portal allows for submission of hospitalized cases using spreadsheets or flat files. Please choose the option for uploading COVID-19 hospitalization information via a csv or excel file.
Reporting for community settings:
Certain community settings (long-term care, K-12 schools, child care, corrections facilities, and shelters) may be required to follow specific reporting protocol and report COVID-19 to MDH. Who is required to report
- All disease reporters. This includes health care providers, clinics, hospitals, and medical laboratories. It also includes long-term care facilities, pre-K-12 schools and child care, institutes of higher education, correctional facilities, and shelters.
